Parent Information
Physicals need to be turned in before an athlete can legally participate in track. Please schedule accordingly. Also be aware if your child's physical expires during the season you will need to update it and turn in another form.
Permission to Practice Form
This form is required by the school district in order for an athlete to practice with the team. One form is required per year per athlete. If your child has already participated in a sport this school year then you do not need to fill out another.
